Instructions for using pdf version of R3 Committee Report form

  1. When you click on the link to the form it will open in your default pdf reader. If that is Adobe Reader, you will get a message:

Click “Sign” to fill out and sign this form. When you are done, you can save a copy by clicking “Done Signing.”

  1. So, click “sign.”
  2. Click on each line, filling in the blanks. The space you have will hold what you need to put in each box, no matter how long that is. You will see the text get smaller to make space for more.
  3. When you finish with the form, you will need to have a signature to place on the document. This is simply a necessity to use this method of submitting the report form, and we don’t care who signs it or what it says.
    • Click “place signature.”
    • If the person on whose computer you are preparing the form already has a signature established, place it at the bottom of the form, below the last box, “Accomplishments since last assembly.
    • If you don’t have a signature created, click on the top right create one, typing (or drawing) a signature – which can be the committee name if you wish. The signature simply has no purpose for Region 3 usage.
  1. You can then save the file to your computer and deliver it to the Recording Secretary on a thumb drive, or you may email it to [email protected].
    • You may email the saved pdf form as your normally would through your mail server.
    • If you use the email-sending device generated by this form, type in your email address, then[email protected], and complete as much of the rest as you wish. You will have to click that you have read and agree to the EchoSign Terms of Use and Privacy Policy. Then sign in to EchoSign, which you can do easily and free, and if you use a Google identify it works quite smoothly.
